4. To obtain a permit, a person shall provide the secretary with the following papers:(a) an application for admittance to the Order, duly completed and in accordance with the form in Schedule 1;
(b) an attestation that he holds a diploma recognized by the Government pursuant to the first paragraph of section 184 of the Professional Code (chapter C-26) or recognized as equivalent by the Order, or training recognized as equivalent by the Order;
(c) complete official transcripts of his marks at the college or university level;
(d) proof, where required by law, that his knowledge of the official language complies with the standards established by Government regulation for that purpose; and
(e) a certified cheque or money order in the amount of $75 as fees for opening the record.
